Challenges

When this aged care provider approached Extranet Systems, they were running 3,000 users on ageing on-premises infrastructure that was approaching end of life, across facilities where system failures directly impacted resident care and clinical operations.

Every hardware failure meant unplanned downtime across systems that could not afford to go offline. A like-for-like infrastructure refresh would lock them into another decade of the same model at significant capital cost. The risks were clear:

  • Hardware failures causing unplanned downtime across clinical and administrative systems
  • Resident care, medication management, and family communications disrupted by outages
  • Significant capital replacement costs with no long-term scalability
  • No elasticity to scale infrastructure as new facilities came online

Our Approach

Our first priority was understanding their environment, not pitching a migration. Before we recommended a single change, we spent time mapping their infrastructure, their clinical dependencies, and the operational risk of any downtime across residential aged care facilities.

What became clear was that the solution was not a straight lift and shift to the cloud. It was a hybrid architecture where the right workloads moved to AWS and Microsoft 365, and resident-facing systems stayed on refreshed Cisco infrastructure where latency demanded it. No unnecessary complexity. No over-engineering. This meant the provider got exactly what they needed to eliminate downtime risk, reduce long-term costs, and scale as new facilities came online.

Strategy and Execution

We worked in focused phases with clear deliverables at each stage. The provider could see progress against their infrastructure roadmap from week one. Nothing was deployed without a mapped operational requirement behind it.

Phase 1, Infrastructure assessment and architecture design

We reviewed their existing environment, documented every dependency across clinical and administrative systems, and designed a hybrid architecture that balanced latency requirements, compliance obligations, and long-term cost efficiency.

Phase 2, Hybrid infrastructure deployment

Cisco-based on-premises infrastructure was refreshed for resident-facing systems where latency demanded it. Critical applications were modernised and migrated to AWS. Microsoft 365 was deployed across the workforce for productivity and collaboration with minimal disruption to operations.

Phase 3, Cloud optimisation and ongoing monitoring

Cloud spend was actively optimised through right-sizing, reserved capacity, and continuous monitoring. The provider was not simply lifted and shifted. Every workload was placed where it performed best and cost least over the long term.

Outcomes

The impact was visible before the final phase closed. A provider that had been managing failing infrastructure across care facilities now had a resilient, monitored environment that protected both operations and resident care from day one of go-live.
